Opening Summary
An opening summary begins the initial case narrative recording, after the completion of the NCFAS G+R intake field, and Written Service Agreement, CD-14B. This should include the reason for case opening, in addition to any relevant information that pertains to the family’s presenting and underlying problems if this information is not clearly described in the NCFAS G+R intake field. The Opening Summary should not replicate information already contained in the NCFAS G+R, but may supplement and expand on the descriptive information contained in this tool. Diligent search efforts to locate the absent parent(s), relatives, and kin should also be documented.
The sections below are mandatory in FACES for FCS and AC opening summaries. See Attachments A and B for case examples.
