Due to requirements that registered family
home providers complete a background screening and provide proof of social
security number, date of birth and Tuberculosis test, two new child care
forms have been developed.
Family Child Care Provider Notice of Registration Approval/Rejection (IM-94A): The Family Child Care Provider Notice of Registration Approval/Rejection, (IM-94A), is used to notify the registered family child care provider applicant of:
The IM-94A is used to notify new providers and providers whose registration lapsed the results of the background screening. There are no comparable FAMIS forms as the provider may not have a vendor number or will not be open in the FAMIS Provider Maintenance system. An electronic form is currently under development and will be available to county offices when completed. If the provider is approved, the following

Parental Notice of Family Child Care Provider Registration Approval/Rejection (IM-94B): The Parental Notice of Family Child Care Provider Registration Approval/Rejection, (IM-94B), is used to notify the child care assistance applicant/recipient that the person they have selected to provide child care has been approved or rejected as a registered family child care provider. It is also used to notify the child care assistance applicant/recipient if the requested provider has failed to supply proof of their social security number, date of birth or Tuberculosis test. Discontinue use of the IM-32CC for the purpose of notifying a child care assistance applicant/recipient that the requested family child care provider has been rejected. been rejected, send the appropriate provider registration forms as noted

A copy of each form and instructions are attached to this memorandum. Continue use of the Notice of Adverse Action (IM-80) for open registered providers. FAMIS is developing new termination reason codes for registration closings due to background screening issues. When these codes are in place, an adverse action notice will be system generated to the provider. |
