FORMS MANUAL REVISION #23: IM-30B
 
DISCUSSION:
Attached is a copy of the revised IM-30B.  This form revision accommodates the budgeting procedure for a Supplemental Nursing Care grant when there is Supported Employment income. Memorandum IM-#44 dated March 10, 2000, discusses this budgeting change.

Use the IM-30B form when determining a:

  • Surplus for nursing facilities (NF), state mental hospital care (MHC) and institutions for the mentally retarded (IMR),

    •  
  • Supplemental Payment (SP) determination of the eligible income level (EIL) and grant, or

    •  
  • Supplemental Nursing Care grant (SNC)
 
NECESSARY ACTION:
  • Discuss this memorandum with all appropriate staff.

    •  
  • Upon depletion of existing supply, begin using the revised IM30-B form.

    •  
  • File the revised form and instructions in the forms manual.
