DISCUSSION:
Applicants and recipients need to clearly understand that eligibility for Medical Assistance for Families (MAF) is no longer tied to eligibility for Temporary Assistance.  To assist with this the IM-32MAF (Approval Notice) and IM-33MAF (Case Action Notice) have been produced to notify applicants for or recipients of Temporary Assistance and/or MAF of approval or other case actions.   These forms have separate sections for Temporary Assistance and MAF which allows staff to address eligibility for each type of assistance.  In addition, the IM-80 has been revised to require staff to address the separate eligibility for cash and medical coverage.  The IM-32 and IM-33 are now to be used only for the adult Medicaid programs (BP, GR, MA, SAB, SNC, etc.)

NOTE:  The IM-33MAF will not be used when adding a person to a Temporary Assistance or MAF case.   Treat this action as an approval and use the IM-32MAF.

An initial supply of these forms is being sent to county offices.  Upon receipt, begin using IM-32MAF and IM-33MAF for Temporary Assistance and/or MAF cases.  Begin using the revised IM-80 for all programs except those that require the IM-80MC.  Order additional forms from the warehouse using normal procedures.

Due to the number of notices produced over the past year, we have created the attached reference guide that identifies the appropriate notice for each type of assistance.

 
NECESSARY ACTION:
  • Share this memorandum with appropriate staff.

  • Begin using the IM-32MAF, IM-33MAF and revised IM-80 upon receipt.

  •  
  • File the new forms, instructions and revisions in the forms manual.
