DISCUSSION:
The form Family Information Record-Attachment (IM-309A) is being revised to reflect changes due to the Workforce Investment Act of 1998.  Employment Security/Jobs Services is entitled Division of Workforce Development and JTPA Private Industry Council is entitled Workforce Investment Board.

No changes have been made to the form's instructions.

Use the current supply of the IM-309A to depletion.  The revised IM-309A may be requested from the warehouse using normal ordering procedures.

  • Review this memorandum with appropriate staff;

  • File the attached revised IM-309A form in the IM Forms Manual behind the IM-309A instructions.
