The mailing address section is now labeled Section A.

Instructions for Section B (Household Information) now state the social security number is required only for persons applying for MC+ coverage.

Section B instructions also explain that race and ethnic group information is optional.

Instructions for Section E now state that failure to cooperate with Division of Child Support Enforcement does not effect a child's eligibility for MC+ coverage, but it may affect a parent's eligibility.

The "Information Needed" section no longer states Social Security cards may be needed prior to approving the application.

In Section A the word, "applicant" has been because the caretaker completing the application may only be applying for MC+ coverage for other family members.

In Section F, a statement has been added to explain that a different application must be completed to apply for coverage based on being age 65, blind, or disabled.

• Discuss this memorandum with all appropriate staff. 

 
• Begin using the IM-1UA upon receipt. 

 
• Destroy/recycle 08/99 or earlier versions of the IM-1UA.

 
• File the revised IM-1UA with instructions in the Forms Manual
