DISCUSSION:
The MC+ application form, IM-1UA, revision date 7/2000, is available in Spanish text.  This is the translation of English version with a revision date of 6/2000.  An initial distribution is not being done at the present time.  The form is available at the distribution warehouse.  Request form number MO 886-3688 (7/00).

A supply of the Spanish IM-1UA may be furnished to local community groups who work with the Spanish speaking population.
 
NECESSARY ACTION:
  • Share this memorandum with appropriate staff as soon as possible.

  • Begin using the Spanish IM-1UA (2/00) for Spanish speaking customers.

  • File the Spanish version IM-1UA in the Forms Manual behind the English version.
