OBSOLETE INDIVIDUAL STRENGTHS ASSESSMENT (IM-312) FORMS MANUAL REVISION #28
 
DISCUSSION:
The purpose of this memorandum is to make obsolete the use of the Individual Strengths Assessment (IM-312).  The Family Information Record (IM-309), which  was implemented in March 2000, serves the same purpose as the IM-312.  The IM-309 assists in screening for individual' strengths and barriers and aids in developing the Family Self-Sufficiency Pact (IM-300).

Upon receipt of this memorandum discontinue using the IM-312 for case management and destroy supply of forms.
 
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.

  • Discontinue using the IM-312.

    •  
  • Remove IM-312 form and instructions from the Forms Manual.

    •  
  • Destroy supply of IM-312s.
