REVISED MC+ APPROVAL LETTER
 FORMS MANUAL REVISION #31
 IM-32MC FORM AND INSTRUCTIONS
 
DISCUSSION:
The IM-32MC, approval letter for MC+ children with income up to 225% of the Federal Poverty Level and custodial parents, has been revised.  The following changes have been made to the letter:

An IMPORTANT NOTICE section now addresses appeal rights of the claimant.  It explains that if a person is not eligible for unrestricted or no-cost coverage, the reason is because the family's income exceeds the limit for that type of coverage.

A non-emergency medical transportation, (NEMT) box has been added to identify persons eligible for this service.  The instructions for the IM-32MC have been revised to reflect the change in the form. 
 
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff.

    •  
  • Begin using the new IM-32MC upon receipt.

    •  
  • Destroy all old paper copies of the IM-32MC.

    •  
  • File the IM-32MC and instructions in the Forms Manual.

    •  
  • Do not use the electronic IM-32MC until the new revised form has been downloaded onto computer.

    •  
  • Order additional supplies of IM-32MC from the warehouse as needed.
