MC+ IMPORTANT INFORMATION FORMS REV #32: MC-4
 
DISCUSSION:
The MC+ Important Information (MC-4) form has been revised.  The MC-4 now reflects that hospitalization coverage is available to the MC+ recipients.  This is not a new service, but one that was inadvertently left off the previous version of the form.  The income guidelines for MC+ have been removed from the back of the MC-4.

An initial supply of the revised form has been sent to county offices for use.  Additional copies are available at the warehouse.
 
NECESSARY ACTION:
  • Share this memorandum with staff.

    •  
  • Begin using the MC-4 upon receipt.

    •  
  • Destroy any outdated MC-4.

    •  
  • File the revised MC-4 in the Forms Manual
