IM-32A (OFFICE GENERATED), NOTICE OF SHORTENED CERTIFICATION/EXPIRATION OF FOOD STAMP BENEFITS FORMS MANUAL REVISION #37: IM-32A, NOTICE OF SHORTENED CERTIFICATION/EXPIRATION OF FOOD STAMP BENEFITS
 
DISCUSSION:
Form IM-32A, Notice of Shortened Certification/Expiration of Food Stamp Benefits, is revised. Use the revised IM-32A for households whose certification is shortened due to failure to provide verification required for a reported change.  Follow policy in IMNL 1140.005.30 regarding shortening a household s certification period.
 
IM-112 FORM

Policy for using the Notice of Expiration (NOE) on the IM-112 form remains the same.  Continue using the NOE on the IM-112 for:

  • a household approved for only one month,
  • a household approved in the last month of certification, or
  • a household approved after the third to last working day in the month prior to the end of the certification period.
FS-1 
 
With each notice of shortened certification (IM-32A), enclose an FS-1, IM-3, IM-31B and IM-31C.  Also enclose a return, self-addressed envelope containing the name of the worker and/or caseload number.

The IM-32A, Notice of Shortened Certification/Expiration of Food Stamp Benefits (08/00), can be ordered from the warehouse using normal ordering procedures.  Upon receipt of the revised IM-32A (08/00), destroy/recycle the previous version with a revision date of 01/97.
 
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff.

    •  

    Enclose a food stamp application, FS-1 (08/99), along with an IM-3,   IM-31B, IM-31C and a self-addressed, return envelope when using the IM-32A form.
     

  • Destroy/recycle the previous version of the IM-32A with a revision date of 01/97.

  • File the revised IM-32A and instructions (08/00) in the IM Forms Manual.
