IM-159  11/07/00  CONTRACTED CASE MGT CONTRACT

CONTRACTED CASE MANAGEMENT CONTRACT
 
DISCUSSION:
Effective November 13, 2000, all referrals and outreach will cease for the Contracted Case Management and Structured Family Assistance Program (SFAP), except for Jackson County.  Case management activities should continue with participants who are actively engaged with the contract provider through January 31, 2001.  This will allow them time to place individuals in full-time unsubsidized employment.

As our agency learns more about the needs of families receiving Temporary Assistance, resources and services must be adjusted to support those needs.  Therefore, the Division of Family Services will be releasing a new Request for Proposal to pilot services to families who have been difficult to engage.  Further information will be given to all county offices as decisions are made.

Local offices will need to work with each provider to transition their closed/active records back to DFS.  Please contact Debbie Bower, Central Office at 573-751-9488 if you would like assistance in developing a plan.
 
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.

  • Cease referrals to Contracted Case Management Providers effective November 13, 2000.
