CASE MANAGEMENT DESK REFERENCE GUIDE
 
DISCUSSION:
The attached Case Management Desk Reference Guide was prepared for case management staff by Team Self-Sufficiency (TSS).

Staff from all areas submitted flow charts, desk references, and charts.  The team reviewed and has compiled that information for easy reference.  Team Self-Sufficiency would like to thank IM staff and the training unit for their hard work in developing references and submitting them to share with workers statewide.

Team Self-Sufficiency is committed to keeping the desk references current.  Revisions will be provided when necessary.

Following are suggestions of other desk references that have been provided in the past that may be added to this booklet for easy reference:

  • Options For Assisting Relative (GAFP)-Memorandum CS99-30/ IM-#115 dated 8-28-99,
  • What to Verify-Memorandum IM-#135 dated 9-24-99,
  • Contracted Case Management Providers Caseload Listing & WtW Comparable Component Listing-Memorandum IM-#138 dated 9-28-99.
 
NECESSARY ACTION:
  • Discuss this memorandum and the Case Management Desk Reference Guide with staff.

  • Begin using the desk references immediately.
