MC+ INCOME GUIDELINES FORM
FORMS MANUAL REVISION #42  MC-4A
 
DISCUSSION:
The MC+ Income Guidelines form (MC-4A) has been updated to reflect the current income limits.  The Medical Assistance for Families (MAF) income guidelines has been added to the Custodial Parent section as a no-cost group.

An initial supply of the revised form has been sent to county offices for use.
 
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.
  •  Begin using the revised MC-4A upon receipt
  • Destroy the previous version of the MC-4A with a revision date of 04/99.
  • File the revised MC-4A in the Forms Manual.
