IM-189  12/27/00  UNDELIVERED/BAD ADDRESS CARD REPORT

UNDELIVERED/BAD ADDRESS CARD REPORT, 
REPORT NO. ESDDY041-01

DISCUSSION:
Report No. ESDDY041-01 "Undelivered Cards Report" is changed.  The revised report is now titled "Undelivered/Bad Address Card Report" and now includes all Benefit Security® Cards that are canceled using code "BA" (Bad Address).  The new cancellation code "BA" was introduced in Memorandum IM-#162, dated 11/14/2000. 

When county staff receive this report, they must review the information in the Administrative Terminal (MOP) and the case record, then contact the household to verify the correct address.  Staff must then take the following action.

  • If the address in MOP is correct, but the household did not receive the previous Benefit Security® Card when mailed to that address, reissue a card to the county office.
  • In EBTU, enter a "C" in the action field, an "O" in the mailing field, and a "Y" in the reissue card field.  Press enter.
  •  A message will appear that the card must be canceled in MOP.  Codes A6 and BA are cancellation codes, so press enter a second time.
  •  The message will read, "Record Updated.  Enter next DCN."
  •  If the address in MOP is incorrect, change the address in FSU5 and/or IMU5, as appropriate.

  •  
    •  In EBTU, enter a "C" in the action field, and a "Y" in the reissue card field.  Press enter.
    • A message will appear that the card must be canceled in MOP.  Codes A6 and BA are cancellation codes, so press enter a second time.
    • The message will read, "Record Updated.  Enter next DCN."
 
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff. 

  •  
  • Verify correct addresses for all cases listed on Report No. ESDDY041-01 and reissue the Benefit Security® Card in EBTU as soon as possible.
  • File Report No. ESDDY041-01 and the forms instructions in the Forms Manual.
