IM-20  01/27/00  FORMS MAN. REVISION #4: IM-19 & INSTRUCTIONS

FORMS MANUAL REVISION # 4:  IM-19 AND INSTRUCTIONS
 
DISCUSSION:
The IM-19 and instructions are revised to reflect instruction given to staff in Memorandum IM-#48 dated April 19, 1999.  The IM-19 is now a four-part form printed on NCR paper.

Use the IM-19 for transportation and other expenses related to food stamp Direct Job Placement (DJP) activities only.  Do NOT use the IM-19 for Temporary Assistance, Work First Transportation Reimbursement.  Do not refer to Work First on the Warrant Request (DBF-14) submitted with the IM-19 requesting payment for food stamp DJP transportation reimbursement.
 
NECESSARY ACTION:
  • Discuss this memorandum with appropriate staff.

    •  
  • Upon depletion of your current supply of the IM-19, begin using the new form.

    •  
  • File the revised IM-19 (12/99) and instructions in the Forms Manual.
