IM-318 REVISION, FORMS MANUAL REVISION #6
 
DISCUSSION:
The form IM-318 Work Site Agreement is revised to reflect changes in Senate Bill 387 and to correct signature line of Work Site Representative.

No changes have been made to the form's instructions.

The revised form is being printed and upon completion, an initial supply will be sent to county offices.  Upon receipt of the revised IM-318, destroy all former revisions.
 
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.

  • Destroy all former revisions of the IM-318 upon receipt of an initial supply of the revised form.

    •  
  • File the revised IM-318 form in the IM Forms Manual.
