The Healthy Children & Youth Program
pamphlet has been revised to reflect current terminology and managed care
issues. Send this pamphlet with all Medicaid and MC+ approval
letters when a person under age 21 is being approved.
With this revision, the IM-50, Healthy
Children and Youth, is obsolete. Information from this form is now
included on the IM-
Use normal procedures to order the revised pamphlet from the warehouse. Destroy your current supply of the pamphlet and begin using the 01/00 revision immediately. Approval notices for Medical Assistance for Families (MAF) or any other MC+ program must include the Important Information sheet (MC-4) and Healthy Children & Youth pamphlet for individuals under 21 years of age. No other information pamphlets are to be included with these notices, unless the family is also being approved for Temporary Assistance. If being approved for Temporary Assistance and MAF, the following information should be included with the approval notification: Healthy Children and Youth
The following pamphlets should be discussed and provided to the individual applying for Temporary Assistance during the application interview. Division of Child Support Enforcement,
What's This About Child Support and What It Means to Establish Paternity
for your baby. (if applicable).
