Dear Parents,

In the past, your family received cash and healthcare coverage through the Division of Family Services.  We want you to know, families can get healthcare coverage without getting cash benefits.  The income limits are much higher and there are no time limits for getting healthcare coverage like there are for cash benefits.

Take a moment to look at the income limits shown on the back of this letter.  These income limits allow families to be employed and still get healthcare coverage.  Many families do not get health insurance through their employment, so it is especially important for uninsured families to apply for this coverage.  Coverage includes hospital care, visits to the doctor, medicine, and other medically necessary services.

To apply for this healthcare coverage, fill out the enclosed MC+ application and mail it to the MC+ Service Center that serves your area.  The date we receive your application will be the date of application.  You will be notified once an eligibility determination is completed.

If you have questions about this MC+ healthcare coverage program or about filling out the MC+ application, please call our MC+ Service Center toll-free at 1-888-275-5908.

       Sincerely,

       Denise Cross
