   FORMS MANUAL REVISION # 19
 
DISCUSSION:
The form Supportive Services Payment Request (IM-317) is being revised to reflect the reference of the DOF-14 to DBF-14.

No changes have been made to the form's instructions.

Use the current supply of the IM-317 to depletion.  The revised 
IM-317 may be requested from the warehouse using normal ordering procedures.

  • Review this memorandum with appropriate staff.
