Effective with the June mailing of the
Notice of Expiration (NOE) and FS-1, we are enclosing a blue window envelope.
The purpose of the blue window envelope
is two-fold:
-
Applicants can use the envelope to return
the NOE and food stamp application by mail and,
-
Staff can quickly identify the food stamp
application when it is received in the county office.
To assist households with using the blue window
envelope to return the FS-1, it was necessary to revise the Notice of Expiration
(NOE). The changes are as follows:
-
On the right side of the form, the NOE now
includes the mailing address for the county office. The street address
of the county office is shown in the upper left corner of the NOE. If there
is a discrepancy in either county office address, notify IM Data Management
to have the address corrected. Please make sure the household's mailing
address in FSU5 is Code-1 compliant and the correct caseload is assigned
for each household.
-
The last paragraph of the NOE now includes
a statement advising the household to make sure the return address appears
in the window of the enclosed envelope.
Applicants must continue to return the NOE
with the application as it contains the information necessary for the window
envelope to be correctly addressed and sorted. The NOE and the FS-1 are
pre-folded to fit in the return envelope. Please assist individuals who
may call the county office with questions about using the blue window envelope
and encourage him/her to use the envelope when returning the FS-1 by mail.
As mail is received in the county office,
please process the blue envelopes IMMEDIATELY. This will enable staff to
screen the mailed-in food stamp applications for expedited service without
delay.
