WORK SITE AGREEMENT (IM-318); FORMS MANUAL REVISION #32
 
DISCUSSION:
Attached is a copy of the revised Work Site Agreement (IM-318).

A participant whose work activity is CWEP/AWEP is protected by Workers Compensation as any other employee at that work site.  An employer accepts this responsibility when signing the Work Site Agreement (IM-318).  A statement regarding Workers Compensation has been added to number one under "Work Site agrees to."
 
NECESSARY ACTION:
  • Discuss this memorandum with all appropriate staff.
  • Destroy existing supply of Work Site Agreements when the revised Work Site Agreements are received.
  • Remove form IM-318 dated 2/00 from the Forms Manual and replace with the revised Work Site Agreement dated 3/01.
