FORMS MANUAL REVISION #33:  IM-39, REQUEST FOR EMPLOYMENT SECURITY INFORMATION - OUTSIDE STATE OF MISSOURI
 
DISCUSSION:
Form IM-39, Request for Employment Security Information - Outside State of Missouri is revised to change the mailing address on the form. Effective immediately, forward the IM-39 to Income Maintenance, FAMIS Unit, 1014 Madison Street, Jefferson City, MO  65101.

Attached to this memorandum is a copy of the new IM-39 (02/01) and the revised instructions.  Use the 08/87 version of the IM-39 to depletion.  Supplies of this form can be obtained from the warehouse using normal ordering procedures.
 
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff.
  • Use the 08/87 version of the IM-39 to depletion and send it to the address listed above.
  • Order a new supply of the IM-39 (08/01) as needed using normal ordering procedures.
  • File the revised IM-39 (08/01) and instructions in the IM Forms Manual.
