IM-31A, REQUEST FOR INFORMATION
FORMS MANUAL REVISION # 1 : IM-31A, REQUEST FOR INFORMATION AND INSTRUCTIONS
 
DISCUSSION:
Form IM-31A, Request for Information, is revised.  The revisions to the form are as follows:
  • In the first paragraph, the year is changed to accommodate 2000.
  • Part III, Proof of all Income, the word "babysitting" is replaced by "child care".
  • Part VI, the title is changed to read "Information About Non-Custodial Parent(s).
  • The word "Case Manager" is added to Caseworker.
Please note for MAF/MC+ applications/reinvestigations, do not use form IM-31A.  The correct form is the IM-31A (MC+).

Attached to this memorandum is a copy of the new IM-31A (12/00).  Use the 07/94 version of the IM-31A to depletion.  Supplies of this form can be obtained from the warehouse using normal ordering procedures.

 
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff.
  • Use the 07/94 version until depletion.
  • Order a new supply of the IM-31A using normal ordering procedures.
  • File the revised IM-31A (12/00) and instructions in the IM Forms Manual.
