WAGE SUPPLEMENTATION PARTICIPATION AGREEMENT

DISCUSSION:
Form Wage Supplementation Participation Agreement (IM-320) has been changed to eliminate a portion of the second agreement statement that the employee signs addressing child support.  This statement has been eliminated from the form since child support exceeds grant payments will no longer be sent to the payee, effective November 1, 2000.  This is in reference to Email #116, dated October 27, 2000. 
 
NECESSARY ACTION:
  • Review this memorandum with appropriate Income Maintenance staff. 
  • Begin using new IM-320 dated 11-00 as soon as received.  Destroy existing supply of Wage Supplementation Participation Agreements when the revised IM-320s are received. 
  • Remove form IM-320 date 3/98 and replace with the revised Wage Supplementation Participation Agreement date 12/00.
