Effective February 1, 2001 the premium
for MC+ for children with income above 225% will increase to $80 per month
and the prescription co-pay will increase to $9. Provider visits
continue to require a co-pay of $10. Families approved prior to February
1 will be notified in writing by First Health of the premium and prescription
co-pay increase. The premium amount for families approved in January
will depend on when it is paid. If paid in January, it will be $68.
If paid in February, it will be $80.
Approval letters (IM-32PRM) sent in January will need to reflect the two premium and prescription co-pay amounts. The electronic version will be changed to include the following sentence: "Effective February 1, 2001, the premium will increase to $80 per month and the prescription co-pay will increase to $9 ". If the electronic version is not used, please write the sentence on the IM-32PRM at the end of the PREMIUM PAYMENTS paragraph. The IM-32PRM will be revised by February 2, 2001 to reflect the $80 premium and increase in prescription co-pay. This version will be available both in hardcopy and in electronic version. The IM-80MC and IM-33MCC is being revised. The premium amount should be entered in the space provided and the new prescription co-pay of $9 amount is shown. Effective February 1, 2001, affordable refers to health insurance requiring a monthly premium of $106 or less per month. |
