IM-2 RECORDING WORKSHEET (IM-38)  FORMS MANUAL REVISION #15
 
DISCUSSION:
Attached is a copy of the revised IM-2 Recording Worksheet (IM-38) (.pdf 27kb, 2 pgs) and form instructions.  This form revision accommodates the following information.

Section 2 (Resources) 

  • A line for the name of the bank the IM-7 came from has been added,
  • a line for the insurance company's name the IM-9 came from has been added,
  • CSV policy number has been removed, and
  • a section to list the burial policy, funeral home, whether the policy is irrevocable and when the IM-9 was received has been added.
Section 6 - now reads Aged, Blind, Disabled. 
Section 9 - A reference contact is not needed if all factors of eligibility have otherwise been verified.  The section for Reference Recording remains if needed.
Section 11 - VR Treatment or Training and the section on Unemployed Parent have been deleted.
Section 15 - The form IM-50 has been removed.  It now reads HCY Pamphlet..
NECESSARY ACTION:
  • Share this memorandum with appropriate staff.
  • Upon depletion of existing supply, begin using the revised IM-38.
  • New forms may be ordered from the warehouses in Kansas City, St. Louis City, and Jefferson City.
  • Remove the form IM-38 dated 11/98 from the Forms Manual and replace it with IM-38 dated 03/01.
