DISCUSSION:
The Division of Child Support Enforcement (DCSE) has recently had two of their forms translated to Spanish and they are now available in the warehouse.  These two forms are:

DOES YOUR CHILD HAVE A LEGAL FATHER? (CSE-6S)

This is a DCSE pamphlet used for information only. 
 

AFFIDAVIT ACKNOWLEDGING PATERNITY (VS-465S)

The Spanish version is not a legal document.  It is only to be used as a "read only" form.  If the participant is interested in signing an affidavit of paternity, then they must sign on the English version of the Affidavit Acknowledging Paternity (VS-465) in order for this to be a legal document.
 
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.
  • Begin ordering the Spanish versions of these forms to accommodate Spanish speaking individuals.  Use this form as a "read only".
  • File the Spanish versions of these forms in the Forms Manual.  The form instructions will not be translated.
