FIELD 06:

• Case action 08 is removed.

• Case action 10 is removed because IM-105 forms are no longer printed.

• Case action 11 is removed.

• Case action 40 is removed.

• Case action code 38 is added.  Use this code when closing a food stamp case due to death.

• Case action code 36, conversion, is removed.

• Case action code 65 is added.  This code is system generated and is used when an expedited food stamp case approval has not been entered in FSU5 by the worker within two working days.

This field is removed.

FIELD 20, LIVING ARRANGEMENT: 

This field includes the different types of living arrangements for a household.  An entry must be made in this field.  An explanation of the codes used in this field can be found in Memorandum IM-#20, dated 01/31/01.

FIELD 26, DISQUALIFIED HOUSEHOLD MEMBER: 

• Code D (other) is removed.

• Disqualified member code "O" is redefined to read "Temporary Assistance/Case Management Sanction".

• Disqualified member code "Z" is changed to six non-work months.

Type of Assistance code "E" is added and used for expanded categorical eligibility.

FIELD 30, RACE/SEX CODES:

This field is revised to list the new race/sex codes.  This is a non-enterable field.  Information in this field is generated by the system when an application is registered.  When a client declares more than one race code, the system selects Code 07, Multi-Racial. 

FIELD 37 - WORK REGISTRATION EXEMPT FOR HOUSEHOLD MEMBERS:

• Code 01 is revised to read "Care for a child under the age of six".

• Code 16 is redefined to read "Participates in or subject to Temporary Assistance Case Management".

• Code 17 is removed.

•  Code 18 is removed.

Code "I", Ineligible Alien, is added.  This code is used when an alien does not meet the eligibility criteria for the Food Stamp Program.

Code "T", Temporary Immigrant, is added.  This code is used for immigrants lawfully admitted but only for a temporary or specific period of time.  Follow policy outlined in FSM 1105.010.30.

Code "U", Undocumented-Illegal, better defines the illegal immigrant status.

FIELD 52 - COUNTY WAIVER CODE

Code 40, Unemployment rate over 10% is removed. 

Code 42, 15% Exemption is added. 

Attached to this memorandum are the revised pages of the FSU5 instructions.  File the revised pages in the IM Forms Manual with IM-105 instructions.  A copy of the FSU5 code sheet may be obtained by accessing the Intranet, or by accessing the online memorandum and PDF file.  Begin using the revised FSU5 Code Sheet (03/02) upon receipt of this memorandum.
