FS-1B, EXPEDITED SERVICE NOTICE AND INSTRUCTIONS
 
DISCUSSION:
As discussed in Memorandum IM-#219 dated 12/17/01, initial applications, including second month applications, are eligible for expedited service.  This policy change necessitated the revision of form FS-1B, Expedited Service Notice.  The changes are as follows: 
  • Added "initial" to first sentence under "Can You Get Expedited (Emergency) Food Stamps?"
  • "Reason you are not eligible for expedited service" is removed.
  • "Other" is removed.
  • The option to fax a food stamp application back to the county office is added.
A copy of the revised FS-1B is attached to this memorandum.  Staff will be notified when the revised electronic version of the FS-1B is available. 

Use the current version of the FS-1B to depletion.  Order the revised FS-1B (03/02) using normal ordering procedures.  File the revised FS-1B (03/02) and instructions in the IM Forms Manual.
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff.
  •  Use the current version of the FS-1B to depletion.
  •  File the revised FS-1B (03/02) and instructions in the IM Forms Manual.
