AFFIDAVIT ACKNOWLEDGING PATERNITY (VS-465)

DISCUSSION:
On August 28, 2001, the Division of Aging and the Department of Health merged to become the Department of Health and Senior Services (DHSS).

DHSS immediately began updating forms to reflect the change and in January notified the Division of Child Support Enforcement (DCSE) of revisions to the Affidavit Acknowledging Paternity (VS-465).  The changes were minimal:

  • The department name changed in three places and
  • The revision date has been updated from 07-99 to 11/01
Because the changes were minimal, DHSS and its Bureau of Vital Records (BVR) will continue to accept the 07/99 version of the VS-465 until further notice.  BVR will not accept earlier versions of the form.
NECESSARY ACTION:
  • Discuss this memorandum with appropriate staff.
  • Remove all versions of the VS-465 and replace them with the 11/01 version.
  • Remove the forms manual instructions and replace it with the 11/01 version.
  • Destroy any versions of the VS-465 prior to 07/99.
