CORRECTION TO IM MEMORANDUM #5; FORMS MANUAL REVISION #4
DISCUSSION:
IM Memorandum #5 dated January 09, 2002 contains an error.  On page 5 of the IM-1 form instructions, the race codes are incorrect.  Attached is the corrected copy (in Adobe Acrobat pdf) with a revision date of January 2002.
NECESSARY ACTION:
  • Discuss this memorandum with appropriate staff.
  • Destroy the October 2001 revision of page 5 and 6 of the IM-1 form instructions in the Forms Manual.
  • File the January 2002 revision of pages 5 and 6 of the IM-1 form instructions in the Forms Manual.
