IM-98  08/02/02 FAMIS FOOD STAMP AND CHILD CARE CASE FILE

FAMIS FOOD STAMP AND CHILD CARE CASE FILE
 
DISCUSSION:
The Family Assistance Management Information System (FAMIS) is an integrated electronic system and case file that contains food stamp and child care applications, documentation, eligibility determinations, and benefit information.  Storing all this information in an electronic file greatly reduces the amount of information required in the hard copy case file.  Effective with conversion of the Food Stamp Program to FAMIS, combine hard copy case file information for food stamps and child care.  File the FAMIS section in the hard copy case file as follows:
  • File the section in chronological order by action date regardless of the program type.  For example, a food stamp application is taken in August 2002, a child care application is taken in September 2002, and a budget adjustment for both programs is completed in December 2002.
  • For each action, file the applicable documents in this order:
    • the FA-100 Request for Application form (or the FS-1 or IM-1CC if the application was filed by mail), the FA-502 Child Care Recertification form and the FA-102 Interview Summary form;

    •  
    • any form requiring the applicant's signature, such as verification forms or the IM-3 Changes You Must Report form;

    •  
    • hard copy verification and eligibility information,  examples of verification include paystubs or income tax papers, examples of eligibility information include income determinations that are made offline and the final result is entered in FAMIS, such as pre-strike income or farm loss.  Use a plain sheet of paper to record information about determinations made offline;

    •  
    • any additional forms required by either program,  examples include the IM-31 Appointment Letter/Request for Contact form for food stamps or the IM-16 Communications Transmittal form, if applicable;

    •  

      NOTE: Do not print and file any notices generated overnight in the batch process as this information is stored online.  If a hearing is requested, print the notices needed for the hearing from the Document Queue (FMVM) at that time.
       

    • any change report forms returned by the EU, such as the IM-145 Change Report form, or other correspondence from the EU.
The IM-100D Food Stamp Change of Status Summary form is not a required form for food stamp cases in FAMIS.
 
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff.
  • File information in the FAMIS section as described above immediately for the Child Care Program and upon implementation into FAMIS for the Food Stamp Program.
