DISCUSSION:
The IM-4FS, Eat for Health brochure, is revised to include the updated gross income maximums for household sizes 1-8, which are effective October 1, 2008.
This brochure may be obtained from the warehouse using normal ordering procedures. Upon receipt of the revised brochure (09/08), destroy/recycle all previous versions.
The brochure will be available on the Department of Social Services Internet site at https://dss.mo.gov/fsd/fstamp/index.htm
To access the PDF version and/or instructions,
- go to Family Support Division (FSD) Intranet site,
- select Food Stamps,
- under Forms select Forms Manual from the drop-down list,
- select Volume 1, and
- select PDF IM-4, Eat for Health brochure or instructions.
An initial supply of the revised IM-4, Eat for Health brochure is being sent to the county offices. Upon receipt, destroy/recycle all versions prior to 09/08.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Order the brochure from the warehouse using normal ordering procedures
- Upon receipt of the revised brochure (09/08), destroy/recycle all previous versions.
