- REQUEST FOR INTERPRETATION OF POLICY (IM-14) FORM REVISION FORMS MANUAL REVISION #3
DISCUSSION:
The IM-14 Request for Interpretation of Policy form is revised to include:
- a subject line,
- the clearance number,
- the position and county of the preparer, and
- the position and county of the person submitting the request,
- additional space for the question,
- check boxes for the program(s) requesting interpretation, and
- check boxes for the program(s) responding to the request.
The formatting of the form is changed for flow and appearance.
The IM-14 will be available through D:FSForms, or can be downloaded from the Forms Manual, Volume 1. Continue to use the IM-14 to request clarifications of Income Maintenance policy and procedures by sending through approved supervisory levels to Martha.J.Bax@dss.mo.gov.
