- FROM:
- ALYSON CAMPBELL, DIRECTOR
- SUBJECT:
- HOUSEHOLD'S REVIEW OF FOOD STAMP INFORMATION IN AUTOMATED SYSTEMS
DISCUSSION:
Effective immediately, at every application for Food Stamp benefits, the eligibility specialist must:
- give the Eligibility Unit (EU) an opportunity to review information entered into FAMIS during the interview by reviewing the FA-102 Interview Summary, and
- provide the EU a copy of the FA-102 for his/her records.
An FA-102 Interview Summary must be reviewed and printed at every interview, whether in person, or by telephone.
In Person
Print the FA-102 Interview Summary and review with the applicant before obtaining his/her signature. Give him/her a copy of the summary and keep the signed original for the agency's records.
Telephone
Mail two copies of the FA-102 Interview Summary to the applicant, one for signature and one for his/her records. If the summary is returned, file the form in the case record. If the summary is not returned, no action is taken.
Comments should be made in EUMEMROL noting the date the summary was provided to the applicant.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Provide every Food Stamp applicant with a copy of the Interview Summary.
