DEPARTMENT OF SOCIAL SERVICES
FAMILY SUPPORT DIVISION
P.O. BOX 2320
JEFFERSON CITY, MISSOURI
TO:
ALL COUNTY OFFICES
FROM:
ALYSON CAMPBELL, DIRECTOR
SUBJECT:
BENEFIT PAYMENT HISTORY REPORT IM-115 REQUEST FOR FOOD STAMP HOUSEHOLD REPORT
FORMS MANUAL REVISION # 10

DISCUSSION:

Food Stamp and Temporary Assistance benefit payment history is available on the Payment History (FM5F or PAYHIST) screen in FAMIS. After a period of time, the payment history is archived and can no longer be viewed in FAMIS. The archived payment history has been available in Managed Reporting by state office staff only and was retrieved for field staff upon request.

To assist field staff, effective August 10, 2009, the Benefit Payment History Report is available to all domains in Managed Reporting. This report provides information for Food Stamp and Temporary Assistance benefits issued from the date the Eligibility Unit (EU) first received benefits in FAMIS through the last day of the month prior to running the report, including regular payroll issuances available in the current month.

FPAY and IPAY still may be accessed, and continue to display issuance information present in Legacy when the case was converted to FAMIS. Generally this is the 36 months prior to FAMIS conversion, although older information may be present in some cases.

ACCESSING THE BENEFIT PAYMENT HISTORY REPORT

Access to the report is through the user's normal Managed Reporting domain; e.g. eligibility specialists access the report through the Eligibility Specialist Domain. Select the "Benefit Payment History" report. Complete the fields on the Benefit Payment History screen as follows:

NOTE: If no benefits were issued through FAMIS for the chosen months, the message "No HTML Output!" is displayed.

IM-115 REQUEST FOR FOOD STAMP ELIGIBILITY UNIT REPORT IS OBSOLETE

It is no longer necessary to complete the IM-115 REQUEST FOR FOOD STAMP ELIGIBILITY UNIT REPORT or to contact Cole.CoIDM@dss.mo.gov, FAMIS or the Program and Policy Unit to obtain archived Food Stamp or Temporary Assistance payment information. Destroy or recycle all copies of form IM-115 and instructions. Persons submitting IM-115 requests to the Cole Co IDM address will be referred to this memorandum.

EBT TRANSACTION HISTORY

In the past, EBT transaction histories were sometimes provided in lieu of issuance histories when the IM-115 REQUEST FOR FOOD STAMP ELIGIBILITY UNIT REPORT was sent to Cole Co IDM. This will no longer occur. Eligibility specialists have access to the last 180 days of transaction history through ebtEDGE. Contact the appropriate Program and Policy Unit through proper channels if an EBT transaction history older than 180 days is required.
