DISCUSSION:
Effective immediately the IM-61 Social Information Summary form is revised to add an additional field for FAMIS USERID in preparation of implementation of the Adult programs in FAMIS.
A copy of the revised IM-61 form is attached to this memorandum. The revised IM-61 form may be ordered from the warehouse. Replace all previous versions.NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Replace all prior versions.
