IM-31A (ELECTRONIC) REQUEST FOR INFORMATION

DISCUSSION:
At the suggestion of the FA325/IM-31A workgroup, the IM-31A Request for Information form is available in electronic format and can be completed online. Because there are drop down boxes, the electronic version should not be printed and completed by hand as the unused fields do not print. The electronic version does not have boxes to check after printing.
Staff should use the FA325 Request for Information generated in FAMIS whenever possible.
Staff should use the IM-31A SPANISH Request for Information when translation is necessary. The electronic version is not translated into Spanish.
Staff may use the current IM-31A Request for Information or the new electronic version only when an FA325 cannot be generated through FAMIS.
The IM-31A ELECTRONIC includes the following changes:
- A drop down box is added to choose the program for the information requested.
- The requested information is divided into the three main areas of Identification,
Resources, and Income. Each selection has a dropdown menu to select requested
information. The field to the right provides space for additional information
to be typed if needed.
NOTE:Only the information requested displays on the final printed document.
- A large "OTHER" section is added to type instructions and requests not included in the drop down selections.
The IM-31A ELECTRONIC will be uploaded soon. To complete the IM-31A ELECTRONIC Request for Information,
- go to D:\FSFORMS and
- select the IM-31A ELECTRONIC.
