DISCUSSION:
Effective immediately the IM-3A Review Notice and IM-2D Review-Eligibility Statement forms are revised to update the terminology from reinvestigation to review.
The IM-3A and IM-2D PDF and WORD versions have been updated in the online Forms Manual on the Intranet and additional supplies may be ordered through the Warehouse. Replace all previous versions. The IM-3A and IM-2D Forms are to be used for reviews of Adult MO HealthNet cases for counties that have not yet converted to FAMIS and for Adult MO HealthNet programs requiring annual review that have not converted to FAMIS including MO HealthNet for Disabled Children, vendor for children residing in JCAHO facilities, and Supplemental Aid to the Blind Vendor programs.
NECESSARY ACTION:
- Review this memorandum with appropriate staff immediately.
- Replace all prior versions.
