DISCUSSION:
November 1, 2010 the Department of Mental Health (DMH) implemented the Disease Management 3700 (DM 3700) project. This memorandum includes information on:
- Purpose of the DM3700 project;
- Eligibility requirements for the DM3700 project;
- Division of Comprehensive Psychiatric Services (CPS) Responsibilities; and
- Family Support Division (FSD) responsibilities.
Purpose of the DM3700 Project
The purpose of this project is to target individuals with chronic medical conditions and provide necessary services to assist these individual to manage their medical/psychiatric conditions. The main goal of this project is to improve medical services that participants receive through coordination with providers and community support while containing medical expenses.
Eligibility Requirements for the DM3700 Project
To be eligible for this program an individual must meet the following criteria.
- $30,000 or greater in combined Medicaid pharmacy and medical costs between June 2009 and May 2010
- A diagnosis of Schizophrenia, Schizoaffective Disorder, Bipolar Disorder, or Major Depression-Recurrent
- Are not current DMH clients
- Have no medical claims between June 2009 and May 2010 for hospice, dialysis, hemophilia, ICFIMR
- Have no nursing home claims between March and May 2010
Division of Comprehensive Psychiatric Services (CPS) Responsibilities
The Division of Comprehensive Psychiatric Services (CPS) through its 21 administrative agents (CMHC’s) along with six affiliate agencies will manage the outreach and enrollment process. The affiliate agencies are Preferred Family Healthcare, Comprehensive Health, New Horizons, Adapt, Places for People, and Independence Center.
Once identified for the project a specific provider will be assigned to the participant to find them and offer services and assistance. The providers will develop a healthcare management plan for each individual. The plans may include services such as:
- Mental health and substance abuse treatment
- Specialized medical care
- Scheduling and keeping medical appointments
- Coordinating care between multiple healthcare providers
- Assisting in obtaining and maintaining safe and affordable housing
- Assisting in obtaining available social safety net benefits in their communities.
FSD RESPONSIBILITES
FSD staff will be responsible for assisting the CPS administrative agents and the six affiliate agencies in obtaining current addresses for FSD participants that have been identified to participate in the program.
Staff should become familiar with this program in the event a participant calls and has questions about the project.NECESSARY ACTION:
