DISCUSSION:

This memorandum introduces procedures for handling internal inspections of the field offices and the handling of Federal Tax Information (FTI).

Field offices are required to submit the attached Internal Inspection Report/Field Office to Central Office annually.  Send the completed report to Stacia Humphrey via email to Stacia.Humphrey@dss.mo.gov. This report will be used to monitor how the office handles the FTI information.  This report must be received in Central Office by June 30th of each year.  The first report is due June 30, 2011.

When completing the Internal Inspection Report/Field Office the office manager or designee must answer each question to the best of their ability. The cover page must be completed with the appropriate information. When answering the questions within the form, place your answer directly below the question. Once the form is completed, the person completing the form must sign and date above “Field Office Official Conducting Internal Inspection” and forward to Central Office.
