DISCUSSION:
The Advance Notice of Your Administrative Disqualification Hearing IM-160 form and the Waiver of Administrative Hearing IM-161 form have been updated to fill- able PDF files.
Staff can use these PDF forms to key-in the data using the keyboard and then print the hard copy version to be mailed to the appropriate hearings office.
