DISCUSSION:
The purpose of this memorandum is to provide revisions to Income Maintenance Manual sections 0105.010.00 Signing the Application and 0105.015.00 Dating the Application, 0105.030.00 Voluntary Withdrawal of Application, and 0105.045.00 Processing IM Applications.
Applications for assistance must be signed. If an unsigned application form is received by the Family Support Division, and the address of the applicant is known, the application form must be returned for signature. Send the original form back to the applicant, guardian, authorized representative or relative for signature with the Signature Request Letter within three business days. If a signed application is not returned, no further action is necessary. Do not keep a copy of the unsigned application form.
The date of application is the date that a signed application is received in the office.
Each applicant has the right to withdraw their application at any time. If the applicant chooses to withdraw, accept the applicant’s oral or written statement that they are withdrawing the application.
When an application is voluntarily withdrawn, clearly indicate that withdrawal was made at the initiative of the applicant. Provide the applicant with appropriate notice that the application was denied.
Income Maintenance Manual section was revised to remove outdated instructions for processing spend down applications.
