DISCUSSION:
The purpose of this memorandum is to introduce the IM-1U MAGI Review form. This form may be given to participants who have misplaced the FA-402 MO HealthNet Review form mailed from the Family Assistance Management Information System (FAMIS) or the IM-1U form mailed from the Missouri Eligibility Determination Enrollment System (MEDES). When participants request a replacement review form, provide them with a copy of this form printed on yellow paper.
NECESSARY ACTION:
- Review memorandum with staff
- Begin using this form immediately, when applicable.
- Use yellow paper when printing this form.
