- INTRODUCTION OF HOME AND COMMUNITY BASED WAIVER (HCB) PROCESSING CENTER
DISCUSSION:
The purpose of this memorandum is to notify staff of the centralized Home and Community Based Waiver (HCB) processing center (PC) located in Poplar Bluff, MO effective May 1, 2015.
This memorandum discusses the following:
- COMPLETING THE IM-54A
- DEPARTMENT OF HEALTH AND SENIOR SERVICES, DIVISION OF SENIOR AND DISABILITY SERVICES RESPONSBILITIES
- HOME AND COMMUNITY BASED WAIVER PROCESSING CENTER RESPONSIBILITIES
COMPLETING THE IM-54A
All staff who receive a request from an individual for HCB will be responsible for completing the IM-54A Home and Community Based Services form and referral process as instructed in Income Maintenance Memo IM-#25, dated 04/08/2015 and clarified in Income Maintenance Memo IM-#72, dated 07/14/2015.
DEPARTMENT OF HEALTH AND SENIOR SERVICES, DIVISION OF SENIOR AND DISABILITY SERVICES RESPONSBILITIES
The Department of Health and Senior Services (DHSS), Division of Senior and Disability Services (DSDS) will complete the assessment and return it to the HCB PC.
HOME AND COMMUNITY BASED WAIVER PROCESSING CENTER RESPONSIBILITIES
Upon receipt of the IM-54A from DHSS, DSDS, the HCB PC eligibility specialists will complete the HCB eligibility determination.
If the HCB referral is denied, the HCB PC will:
- Update the Home and Community Based/PACE Information (HCBINFO, FMJ7) screen;
- Complete the Eligibility Determination Resolution (EDRES, FM3Y);
- Make comments on the Eligibility Unit Member Role (EUMEMROL,FM3Z) screen; and
- Leave the case in the same caseload.
If the HCB referral is approved, the HCB PC will:
- Update the Supercase Address/Supercase Transfer (ADDRESS, FM0A) screen to county 01202 (Butler County);
- Update the Home and Community Based/PACE Information (HCBINFO, FMJ7) screen;
- Complete processing the case;
- Complete the Eligibility Determination Resolution (EDRES, FM3Y);
- Make comments on the Eligibility Unit Member Role (EUMEMROL,FM3Z) screen; and
- Move the case to the appropriate caseload within Butler County FSD.
NOTE: HCB cases who are the community spouse of an active Vendor case maintained in one of the Vendor Units will not be moved to Butler County. These cases will remain in the Vendor Unit.
The HCB PC will maintain all active HCB cases. ES’s in the HCB PC are responsible for maintaining the HCB case record, including but not limited to:
- Completing annual HCB reinvestigations and change reports,
- Responding to inquiries about HCB from participants and their authorized representatives, and
- Completing the Application for State Hearings (IM-87) forms regarding HCB and forwarding to the Hearings Unit.

- Review this memorandum with appropriate staff.
