DISCUSSION:
The Food Stamp Application (FS-1) has been revised. Use the new version of the FS-1 effective October 13, 2015. Destroy/recycle all versions of the FS-1 prior to this revision. The overall layout of this revision will support staff with screening expedited households, incorporate work registration language and plain language. The PDF and Word versions of the FS-1 will be updated on the FSD websites on October 13, 2015.
Effective October 2015, FS-1s mailed out with monthly recertification packets will be printed on green paper instead of white paper. This will assist offices with sorting FS-1s received via mail, the drop box or face-to-face. Any FS-1 received on white paper is an initial application whereas any FS-1 received on green paper is being submitting in conjunction with a recertification.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Contact the warehouse if the new FS-1 is not received by October 9, 2015.
- Destroy/recycle all versions of the FS-1 with a revision prior to October 2015.
- Begin using the revised FS-1 effective October 13, 2015.
