The purpose of this memorandum is to inform staff of a new paper Temporary Assistance (TA) application, the IM-1TA, and procedures to follow for in person and mail-in applications.

New IM-1TA

The new IM-1TA (MO 886-4573) includes all questions asked in the controlled flow in the Family Assistance Management Information System (FAMIS) and incorporates many of the forms currently used when conducting an interactive interview. When the IM-1TA is submitted by an applicant for TA, an interview is not required, however it may be necessary for staff to contact the applicant to clarify information or request additional information to complete the eligibility determination.

The following forms have been incorporated in the IM-1TA application:

Upon receipt of the completed and signed IM-1TA, register the application in FAMIS as a mail-in (MI) application on the REQUEST/FM0G screen. Follow local office procedures regarding handling the TA application for scanning or processing.

This application is available online at https://dss.mo.gov/fsd/temporary-assistance-orientation.htm at the end of the TA Orientation video. It is a fillable PDF, so applicants may complete using the keyboard and print, print and complete by hand, or they may type part of it and handwrite the remainder.

New IM-1TASF

Applicants may continue to apply at FSD Resource Centers in person. If the applicant does not wish to fill out and drop off the IM-1TA, they may complete and sign the IM-1TASF or IM-1 if also applying for other programs and FSD will contact them for the interview. Provide all other necessary paper notices/forms for the TA application and answer any questions the participant may have about the application process. This includes providing the participant with the following forms and informational notices:

In-person Interviews

Applicants may continue to apply at FSD Resource Centers in person and complete interviews with Call Center staff by phone. Staff will continue to follow current procedures used for in-person applications, including obtaining signatures on the Application for Assistance (FA100) or the IM-1 and Interview Summary (FA102).
