THIRD PARTY RESOURCE (TPL-1) FORM REQUIRED FOR CHANGES TO INSURANCE FOR MO HEALTHNET PARTICIPANTS

DISCUSSION:

This memorandum will remind staff that the MO HealthNet Division (MHD) is not notified when new insurance is added or changes are made to existing insurance coverage in the Family Assistance Management System (FAMIS) or the Missouri Eligibility Determination and Enrollment System (MEDES). The Third Party Resource Form (TPL-1) must be completed and submitted to MHD each time insurance coverage is added or changed. This is true for individuals receiving MO HealthNet (MHN) of any type in legacy, FAMIS or MEDES.

The TPL-1 and instructions have been added to the IM Forms Manual Volume II.

NOTE: The instructions are written to Children’s Division staff. Family Support Division staff will use the same form.
