CLARIFICATION FOR CORRECT USE OF THE REQUEST FOR INFORMATION (IM-31A or FA-325) FORM FOR MO HEALTHNET PROGRAMS

DISCUSSION:

The purpose of this memorandum is to clarify the correct use of the IM-31A or FA-325 for MO HealthNet programs. When someone applies or reports a change, only one IM-31A is sent advising the participant of any verification necessary to make an eligibility determination. This form allows the participant 10 days to provide requested verification.

The FSD is only required to send one IM-31A or FA-325 for any MO HealthNet program, including the Aged, Blind, and Disabled programs.

NOTE: This clarification is only for the MO HealthNet programs. All other Income Maintenance programs continue to require two requests for information at application.

If the participant fails to provide the requested verification within the 10 days provided, or fails to request additional time to provide the information, FSD must either reject the application or issue an adverse action to close active assistance.

MAGI Manual section 1805.000.00 is updated to reflect this change.
